From the project exemplar that I reviewed, I learned about theory underscore and support the DNP project together with the application of the Team Strategies and Tools to Enhance Performance and Patient Safety (TeamSTEPPS), in improving the Labor and Delivery setting. Additionally, I understood the application of the Kotter's theory, which includes:

Creating a sense of urgency

Building a guiding team

Establishing a change strategy and vision

Understanding and buy it

Empowering others

Short term wins

Dont give up

Creating a new culture.

Throughout the exemplar, I identified Kotter's theory as an effective tool since it involved a series of stakeholders into planning, and engaging the whole team, together with evaluating the results to establish a culture change indicative of a very steadfast team.

How will you apply this to your DNP project - (Polypharmacy in the elderly)?

With the insight that I have gained through the project exemplar, I will be seeking to include other stakeholders in my DNP project which include, the L&D RN Educators, and Womens Services Clinical Nurse Specialist. The Situation Background Assessment Recommendation (SBAR) tool will then be applied a cross the continuum of care to enhance coordination and communication of the Polypharmacy patient (Anderson, Knestrick., & Rebeca(Eds.), 2014).

What are the role of the DNP in theory, research, and scholarship? (choose one of these roles and discuss).

Research

The DNP research is essential in the health and clinical settings. Nursing is considered to be one of the most assorted profession since it addresses leadership, clinical, public health care needs as well as the public policy. Since, nurses have the vast range of education credential, starting from the associates up to the doctoral degree, therefore making them potential researchers in the DNP research (Sergi, De Rui, Sarti, & Manzato, 2011). The role of the DNP is to research factors that influences, clinical settings between patient and the physicians, leadership in the clinical settings, together with the implementation and repealing of some nursing policies. The DNP research ensures that the patients are provided with the highest quality of health care services to the patient as well as the broader community (Sergi, De Rui, Sarti, & Manzato, 2011).

How do you envision your nursing leadership role upon graduation?

Since the DNP degree is established on the foundation created by the previous nursing experience as well as the Nursing Doctorate (ND) degree, once I graduate, I envision at adhering to addressing the critical skills required to translate evidence-based care into practice, enhancing the system of care, then measuring the outcomes of patients of a particular group and communities (In Anderson, In Knestrick, & In Barroso, 2015). Having graduated with DNP I will be equipped with considerable leadership and administrative skills that will improve the patients' experience and better the population health especially in the project that I am working on, Polypharmacy in the elderly. Furthermore, I will ensure that the relevant stakeholders (example government, caregivers, policy makers etc.) are keen at ensuring that the medical practice is enhanced by ensuring that the elderly are not subject to Polypharmacy, which increases their risks of getting side effects which deteriorate their life (Kleinpell, 2009).
